You never know when a sudden illness may arise or a serious accident happen. Learn how to plan for your future needs so your wishes will be respected and carried out if you can't speak for yourself.

Have you thought about what will be most important to you if you were seriously ill or injured, or if you were near the end of life?  Who will speak for you if you are unable, and have you communicated your beliefs, values and wishes clearly to them?  

Led by Compassionate Communities - Kingston, Canada, this Advance Care Planning session helps you arrange for your future care needs so your values, beliefs and wishes will be respected and carried out.  Learn how choosing a Substitute Decision Maker and communicating clearly your future care needs to them now can allow you to maintain responsibility for your own autonomy, and to avoid family disputes or agonizing dilemmas.
