Learn how choosing a Substitute Decision Maker and communicating clearly your future care needs to them now can allow you to maintain responsibility for your own autonomy, and to avoid family disputes or agonizing dilemmas.
This interactive online program from Compassionate Communities Kingston will be held on Zoom. Registrants will be emailed the link immediately upon registration.
