Learn how choosing a Substitute Decision Maker and communicating clearly your future care needs to them now can allow you to maintain responsibility for your own autonomy, and to avoid family disputes or agonizing dilemmas.
This interactive online program from Compassionate Communities Kingston will be held on Zoom. Registrants will be emailed the link when they register. Participants will need to download the latest version of Zoom in advance to the device they’ll be using during the presentation.
This Introduction meeting is the first of two presentations on this topic. Once you have chosen your Substitute Decision Maker(s) and have begun setting up your Advance Care Plan, please consider registering for the followup presentation Advance Care Planning: Review for Past Attendees in March or April, where you can discuss questions arising from your implementation.
